What is a Chronic Disease Management Plan?
You may have been referred to us by your GP under a Chronic Disease Management Plan (CDMP). This plan gives you a medicare rebate on up to 5 services per calendar year.
A CDMP is for someone with a chronic medical condition which is defined as one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. The CDM items are designed for patients who require a structured approach and to enable GPs to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team.

We can monitor your adherence to the exercise program and can easily modify your existing program as required.
Microsoft Teams is so simple and straightforward. We will send you a link that you click on at the time of your appointment. It will take you to your exercise physiologists’ private appointment that was created just for you.
