Medicare Rebates
MEDICARE

Medicare rebates are available for people with a chronic disease such as arthritis, cancer, osteoporosis etc. if they are referred by a GP with an EPC/CDM (Enhanced Primary Care/Chronic Disease Management). o With this referral, you can receive up to 5 partially funded sessions under Medicare in a calendar year. o Medicare currently contribute a rebate of $58.30 leaving an outstanding out of pocket amount. However, if you have exceeded the threshold on the Medicare safety net you can receive up to 80-100% back. o The Medicare Safety Net is a threshold in which if you spend over a certain amount of verified gap/out of pocket costs in a calendar year, the amount you get back from Medicare increases. You must obtain a referral from your GP (lasts 12-months) prior to your first appointment with us. o The GP will complete this form to accompany your medical history and a consent form for your EP to sign. o These forms are usually faxed to us but feel free to being a hardcopy just in case. o When you have utilised all your sessions, you may then use private health insurance to provide rebate, but they cannot be used together to cover the cost of the appointment. If you have any further questions, feel free to call our lovey reception staff.
